I'm assuming the Quiz Mod, has permissions to set up.
Have you tried checking to be sure you have the appropriate permissions to add new quizzes?
If all else fails, the MOD is quite possibly already installed.
Try refreshing your style imageset, purging the cache, and then refreshing the page in your browser by pressing Ctrl + F5
